FOXO4-DRI vs Humanin

FOXO4-DRI and humanin are both studied in ageing biology, and both act on cell-death pathways, though in opposite directions in the studies reviewed 1,6. FOXO4-DRI is a designed D-retro-inverso peptide that, in cell and mouse studies, disrupts the FOXO4-p53 interaction so that senescent cells undergo apoptosis 1. Humanin is a naturally occurring 24-amino-acid mitochondrial-derived peptide that, in cell studies, blocks pro-apoptotic proteins and protects neurons 6,7,10. Neither has been tested as a treatment in a registered human trial, and neither is FDA-approved 11,12,13.

FOXO4-DRI: what the data says

FOXO4-DRI was described in a 2017 Cell paper that identified FOXO4 as central to the survival of senescent cells 1. The peptide uses D-amino acids in a reversed sequence and is fused to the HIV-TAT sequence to enter cells 1. In senescent human cell cultures (cell data) it caused p53 nuclear exclusion and apoptosis, and the effect was reduced when p53 was knocked down or caspases were blocked 1. In fast-ageing and naturally aged mice (animal data), the authors reported restored fitness, fur density and kidney function 1. A 2025 NMR study mapped its binding to the p53 transactivation domain 2.

Other animal work points in several directions. In aged mice, it was reported to act on senescent Leydig cells and age-related testosterone decline 4. In mice, it reduced collagen deposition in a bleomycin lung-fibrosis model 16. In a 2023 Circulation study, however, removing senescent pulmonary endothelial cells with this peptide or another senolytic worsened pulmonary haemodynamics in mice 3. No human studies were identified 11.

Humanin: what the data says

Humanin was identified in 2001 as a secreted polypeptide that, in cell culture, prevented neuronal death caused by several familial Alzheimer's disease genes and amyloid-beta 5. In cell studies it binds and inhibits the pro-apoptotic proteins Bax and Bid 6 and acts through a receptor complex involving CNTFR-alpha, WSX-1 and gp130 7. In rodents (animal data), brain infusion of humanin and intravenous infusion of analogues changed insulin sensitivity 8.

Human evidence concerns the body's own humanin rather than giving the peptide. Circulating levels were lower with age in humans and mice 8, lower in 20 patients with coronary endothelial dysfunction than in 20 without 9, and rose after acute endurance exercise but not resistance exercise in a 30-person study 10. No interventional human trials were found 12.

Key research differences

  • Direction of effect on apoptosis: FOXO4-DRI promotes apoptosis in senescent cells 1, whereas humanin blocks pro-apoptotic proteins in cell studies 6.
  • Origin: FOXO4-DRI is a designed D-amino-acid peptide 1; humanin occurs naturally in the body 10.
  • Target: FOXO4-DRI acts on the FOXO4-p53 interaction 1,2, while humanin work centres on Bcl-2 family proteins and a cytokine-type receptor complex 6,7.
  • Human data: no human studies of FOXO4-DRI were identified 11, while humanin has observational blood-level studies in people 9,10.
  • Conflicting animal findings: one mouse study reported that senolytic clearance with FOXO4-DRI worsened pulmonary haemodynamics 3; no comparable reversal was located for humanin.

Where the evidence is limited

No study has compared FOXO4-DRI and humanin. FOXO4-DRI evidence is entirely preclinical, much of it from the group that designed the peptide or from authors with a commercial interest in it 1,2. Humanin human data are correlational, measuring natural blood levels rather than testing administration, and animal studies often used modified analogues 8,9,10. Mouse senolytic results also diverge by tissue, so a single animal finding does not generalise 1,3.

Frequently asked questions

What is a senolytic peptide?
FOXO4-DRI is described as a senolytic: in cell and mouse studies it causes senescent cells, which avoid apoptosis through the FOXO4-p53 interaction, to die 1.
Has FOXO4-DRI been tested in people?
No human studies were identified, and a ClinicalTrials.gov search returned no trials of this peptide 11.
Can FOXO4-DRI and humanin be compared directly?
Not on current evidence. No head-to-head study exists, and in the studies reviewed they act in opposite directions on cell death 1,6.
Is clearing senescent cells always favourable in animal studies?
No. In mice, clearing senescent pulmonary endothelial cells with FOXO4-DRI or another senolytic worsened pulmonary haemodynamics 3.
