Semaglutide vs survodutide

Semaglutide is an FDA-approved GLP-1 receptor agonist, and survodutide is an investigational peptide that activates both the GLP-1 receptor and the glucagon receptor 1,2,3. They have been compared directly in a 16-week phase 2 trial in type 2 diabetes that included an open-label semaglutide arm 4, and in obese mice (animal data) 5. No phase 3 head-to-head trial was identified 6.

Semaglutide: what the data says

Semaglutide is FDA-approved for type 2 diabetes, cardiovascular and kidney risk reduction in specified groups, chronic weight management and, under accelerated approval, MASH with moderate to advanced fibrosis 1,7.

  • In STEP 1 (1961 adults without diabetes; human data), mean body-weight change at 68 weeks was -14.9% vs -2.4% with placebo 8.
  • In SELECT (17,604 adults with cardiovascular disease; human data), the primary cardiovascular outcome occurred in 6.5% vs 8.0% with placebo 9.
  • In FLOW (3533 adults with type 2 diabetes and chronic kidney disease; human data), major kidney disease events were 24% lower than with placebo 10.

Survodutide: what the data says

Survodutide has completed phase 3 trials in obesity, type 2 diabetes with obesity, and MASLD 3,11,12.

  • In SYNCHRONIZE-1 (725 adults; human data), mean body-weight change at 76 weeks was -12.2% and -13.0% vs -5.4% with placebo 3.
  • In a 16-week phase 2 trial in 413 people with type 2 diabetes on metformin (human data), HbA1c reductions were similar between a low survodutide dose and open-label semaglutide (-1.46% vs -1.47%), while body-weight change reached -8.7% with survodutide vs -5.3% with semaglutide 4.
  • In obese mice (animal data), survodutide produced larger body-weight reductions than semaglutide at doses giving its maximal response, attributed to added energy expenditure 5.

Key research differences

  • Receptor targets: semaglutide acts only on the GLP-1 receptor, while survodutide also activates the glucagon receptor 1,3.
  • Regulatory status: semaglutide has been FDA-approved since 2017; survodutide had no Drugs@FDA record on 24 October 2026 2,13.
  • Direct phase 2 comparison: in type 2 diabetes over 16 weeks, HbA1c change was similar at a low survodutide dose, and body-weight change was larger at higher survodutide doses than with open-label semaglutide 4.
  • Adverse events in that comparison: reported by 77.8% of survodutide-treated participants vs 52.0% on semaglutide, mainly gastrointestinal 4.
  • Outcome trials: semaglutide has published cardiovascular (SELECT) and kidney (FLOW) outcome trials; no comparable outcome trial results for survodutide were located 9,10.

Where the evidence is limited

The only human head-to-head data come from a 16-week phase 2 trial in which the semaglutide arm was open-label and used doses up to 1.0 mg weekly, so it does not reflect longer-term use or the higher dose approved for weight management 4. The mouse comparison is animal data only 5. The phase 3 trials of each drug used different durations and populations, and each programme was funded by its developer 3,4,8.

Frequently asked questions

Has survodutide been compared with semaglutide?
Yes, in a 16-week phase 2 trial in type 2 diabetes: HbA1c reductions were similar for a low survodutide dose and semaglutide, and body-weight change was larger with higher survodutide doses, with more adverse events 4.
Does survodutide lead to more body-weight change than semaglutide?
The evidence does not settle this. The only human comparison was short, used an open-label semaglutide arm (up to 1.0 mg weekly) and was sponsored by survodutide's developer 4.
Is survodutide FDA-approved?
No. No Drugs@FDA application record was returned on 24 October 2026, and its 2026 phase 3 reports describe it as investigational 3,13.
