Tirzepatide vs survodutide

Tirzepatide and survodutide both activate the GLP-1 receptor, but tirzepatide pairs it with the GIP receptor while survodutide pairs it with the glucagon receptor 1,2. Tirzepatide is FDA-approved as Mounjaro and Zepbound 3. Survodutide is investigational: its phase 3 obesity results were published in 2026, and no Drugs@FDA application record was returned on 24 October 2026 2,4. No head-to-head trial was identified 5.

Tirzepatide: what the data says

Tirzepatide is FDA-approved for type 2 diabetes and cardiovascular risk reduction in high-risk adults with type 2 diabetes (Mounjaro), and for chronic weight management and obstructive sleep apnea in adults with obesity (Zepbound) 1,7.

  • In SURMOUNT-1 (2539 adults without diabetes; human data), mean body-weight change at 72 weeks was -15.0% to -20.9% vs -3.1% with placebo 8.
  • In SURMOUNT-OSA (human data), the apnea-hypopnea index fell by 20.0 and 23.8 more events per hour than with placebo in the two trials 9.

Survodutide: what the data says

Survodutide's evidence comes from a Boehringer Ingelheim programme spanning obesity, type 2 diabetes and liver disease 2,10,12.

  • In SYNCHRONIZE-1 (725 adults; human data), mean body-weight change at 76 weeks was -12.2% and -13.0% vs -5.4% with placebo; no deaths were reported 2.
  • In SYNCHRONIZE-MASLD (216 adults; human data), 84.2% vs 24.3% had at least a 30% fall in MRI-measured liver fat at 48 weeks (efficacy estimand) 11.
  • In a phase 2 MASH trial (human data), improvement in MASH without worsening of fibrosis occurred in 43% to 62% vs 14% with placebo 12.

Key research differences

  • Second receptor target: GIP receptor for tirzepatide vs glucagon receptor for survodutide 1,2.
  • Regulatory status: tirzepatide has FDA approvals from 2022 and 2023, while survodutide had no Drugs@FDA record on 24 October 2026 3,4.
  • Liver focus: survodutide has published MASLD phase 3 and MASH phase 2 trials 11,12, while tirzepatide's FDA labels list diabetes, weight-management, cardiovascular and sleep-apnea indications 1,7.
  • Half-life data: the tirzepatide label gives about 5 days, whereas no numeric human half-life for survodutide was located 1.
  • Developers: tirzepatide trials were funded by Eli Lilly and survodutide trials by Boehringer Ingelheim 2,8.

Where the evidence is limited

No randomized trial has compared tirzepatide and survodutide directly in the sources reviewed 5. SURMOUNT-1 and SYNCHRONIZE-1 differ in duration (72 vs 76 weeks), placebo-group change (-3.1% vs -5.4%) and statistical estimands, so cross-trial differences in percentages are not a reliable guide 2,8.

Frequently asked questions

Does tirzepatide lead to more body-weight change than survodutide?
This cannot be determined from the evidence reviewed. No head-to-head trial exists, and the phase 3 trials of each used different durations and had different placebo responses 2,5,8.
Is survodutide FDA-approved?
No. An openFDA query of Drugs@FDA on 24 October 2026 returned no application record, and its 2026 phase 3 reports call it investigational 2,4.
How do tirzepatide and survodutide differ in mechanism?
Both activate the GLP-1 receptor. Tirzepatide also activates the GIP receptor, while survodutide also activates the glucagon receptor, which in mice was linked to increased energy expenditure 1,6.
