Kisspeptin-10 vs Gonadorelin

Kisspeptin-10 and gonadorelin both act on the reproductive hormone axis, but at different steps and with different regulatory histories 3,9. Kisspeptin-10 is the shortest kisspeptin fragment with full activity; in healthy men, intravenous boluses raised luteinising hormone and continuous infusion raised testosterone 3. Gonadorelin is synthetic gonadotropin-releasing hormone itself, acting directly on the pituitary GnRH receptor, and was approved in the United States as the LUTREPULSE KIT (NDA 019687), whose products are now flagged discontinued in Drugs@FDA 8,9. The FDA places kisspeptin-10 in 503A category 2, bulk substances that raise significant safety risks, and the WADA 2026 list names both kisspeptin analogues and gonadorelin under testosterone-stimulating peptides in males 11,13.

FieldKisspeptin-10Gonadorelin
ClassSynthetic 10-amino-acid fragment of kisspeptin; kisspeptin receptor (KISS1R) agonist 2,3Synthetic decapeptide identical to native gonadotropin-releasing hormone; GnRH receptor agonist 7,9
Origin and structureMinimal kisspeptin sequence retaining full intrinsic activity; the parent peptide was first described as metastin 2,3Native hypothalamic decapeptide that regulates pituitary LH and FSH release; longer-acting analogues were later developed because of its short half-life 7
Proposed mechanismActs upstream of GnRH neurons; a 2007 review describes the kisspeptin system's role in reproduction, and human studies show stimulation of GnRH and so gonadotropin secretion 1,3Acts directly on pituitary GnRH receptors to release LH and FSH 7,9
Half-lifeNot located in the sources reviewedNo value located; a 1991 review describes native GnRH as having low potency and a short half-life, which prompted development of analogues 7
Highest human trial phaseRegistered trials up to phase 2 and 3 in small single-centre studies, for example NCT03771326 (14 men, completed) and NCT07224438 (phase 2) 12Previously FDA-approved for human use under NDA 019687, now discontinued; still used in GnRH stimulation testing and pulsatile ovulation induction 8,5,6
FDA statusNot approved; no Drugs@FDA record for kisspeptin (queried 24 October 2026) 10; listed in 503A category 2, substances that raise significant safety risks, as updated 14 May 2026 11Approved under NDA 019687 (LUTREPULSE KIT, Ferring), with both products flagged Discontinued; current DailyMed gonadorelin labels are veterinary 8,9; not in any 503A nomination category 11
Published human outcomesIn healthy men, boluses raised serum LH and infusion raised LH pulse frequency and testosterone 3; daily 8-hour subcutaneous infusions over 12 days sustained gonadotropin rises in 7 men 4In 12 women with hypothalamic amenorrhoea, pulsatile GnRH produced documented ovulation in 30 of 33 cycles and 10 pregnancies in 7 patients 6
Safety signalsFDA cites immunogenicity risk, peptide-impurity complexity and no or only limited safety information for the proposed routes 13; the trials reviewed reported no adverse effects 14,15A case report describes pituitary apoplexy after TRH and GnRH stimulation testing in a patient with acromegaly 16
WADA statusProhibited at all times: section S2.2.1 names kisspeptin and its agonist analogues under testosterone-stimulating peptides in males 17Prohibited at all times: section S2.2.1 names gonadorelin (GnRH) and its agonist analogues under testosterone-stimulating peptides in males 17
Who funded the evidenceAcademic groups at Imperial College London and Edinburgh; several authors report consultancy with pharmaceutical companies outside the work 3,4,14Clinical and academic groups; the original approval was held by Ferring 6,8

Kisspeptin-10: what the data says

Kisspeptin-10 is the shortest kisspeptin fragment that retains full activity at the kisspeptin receptor, and it acts upstream of GnRH neurons 2,3. In healthy men, intravenous boluses produced a rapid rise in serum LH, and continuous infusion raised LH pulse frequency, pulse size and testosterone 3. A 2026 randomised, single-blinded, placebo-controlled study in 15 healthy men found that continuous subcutaneous infusion over 5 days left gonadotropins similar to vehicle, while daily 8-hour infusions over 12 days sustained gonadotropin increases 4.

Related work used the longer kisspeptin-54 in sexual-desire studies: randomised crossover trials in 32 women and 32 men with hypoactive sexual desire disorder reported changes in brain activity on functional MRI, with penile tumescence changes in the male trial 14,15. The FDA places kisspeptin-10 in 503A category 2, citing immunogenicity risk and no or only limited safety information 13.

Gonadorelin: what the data says

Gonadorelin is synthetic GnRH, the hypothalamic decapeptide that drives pituitary release of LH and FSH 7,9. Because the native peptide has low potency and a short half-life, longer-acting analogues such as leuprolide and nafarelin were developed 7.

Its human uses are long established. Pulsatile GnRH administration by pump is described as a method for inducing ovulation in women with hypothalamic amenorrhoea: in 12 such patients, ovulation was documented in 30 of 33 treatment cycles, with 10 pregnancies across 7 patients 6. GnRH stimulation testing remains a diagnostic tool in precocious puberty, with the caveat that cutoffs vary between studies 5. A case report describes pituitary apoplexy after TRH and GnRH stimulation testing in a patient with acromegaly 16. In the United States, the approved human product under NDA 019687 is flagged discontinued, and the gonadorelin labels now in DailyMed are veterinary 8,9.

Key research differences

  • Site of action: kisspeptin-10 acts upstream, on GnRH neurons, whereas gonadorelin acts directly on pituitary GnRH receptors 3,9.
  • Approval history: gonadorelin was FDA-approved for human use but its products are discontinued, while kisspeptin-10 has never been approved 8,10.
  • FDA compounding standing: kisspeptin-10 is in 503A category 2 for significant safety risks; gonadorelin appears in no nomination category 11,13.
  • Clinical track record: gonadorelin has decades of use in ovulation induction and stimulation testing 5,6, while kisspeptin-10 evidence is small single-centre physiology and early-phase studies 3,4.
  • Both are named on the WADA 2026 list under testosterone-stimulating peptides in males, so neither offers an anti-doping advantage over the other 17.

Where the evidence is limited

No study has compared kisspeptin-10 with gonadorelin directly. The kisspeptin-10 studies are small physiology experiments in healthy men, measuring hormone levels rather than clinical outcomes 3,4. The gonadorelin evidence is older, often uncontrolled clinical series in women with hypothalamic amenorrhoea, and reflects a product no longer marketed for human use in the United States 6,8. The two have also been studied for different purposes, fertility treatment and diagnostic testing versus hormone-axis physiology and sexual desire research 5,14.

Frequently asked questions

Is gonadorelin still available as a human medicine in the US?
The approved human product under NDA 019687 is flagged Discontinued in Drugs@FDA, and the gonadorelin labels now in DailyMed are veterinary 8,9.
What does the FDA say about kisspeptin-10?
It is listed in 503A category 2, bulk substances that raise significant safety risks, citing immunogenicity risk, impurity and characterisation complexity, and no or only limited safety information 11,13.
Are both prohibited in sport?
