The basics

What is tirzepatide, and what is it used for?

Drug label
Tirzepatide is an FDA-approved prescription drug that activates the GIP and GLP-1 receptors. As Mounjaro, it is approved for type 2 diabetes. As Zepbound, it is approved for chronic weight management and for moderate to severe obstructive sleep apnea in adults with obesity.

Mounjaro was first approved in May 2022 and Zepbound in November 2023 3,14. The Mounjaro label also covers reducing major cardiovascular events in adults with type 2 diabetes at high risk 1.

Mounjaro is approved for children 10 years and older with type 2 diabetes; Zepbound is approved for adults 1,2. Amiravo's tirzepatide entry summarises both labels and the main trials.

What is tirzepatide made of, and how does it work in the body?

Drug label
Tirzepatide is a synthetic peptide based on the GIP hormone sequence. A fatty-acid chain binds albumin and stretches its half-life to about 5 days. It raises insulin release and lowers glucagon when glucose is high, reduces food intake and slows stomach emptying.

The label describes non-standard amino acids at positions 2 and 13 and a C20 fatty diacid attached to a lysine 2. Its molecular weight is 4813.53 Da 1.

The label states that tirzepatide increased insulin sensitivity in a clamp study after 28 weeks 1. The slowing of stomach emptying is largest after the first dose and fades over time 1. Half-life engineering is explained in How peptides are engineered to last longer in the body.

Are tirzepatide, Mounjaro and Zepbound the same?

Drug label
Mounjaro and Zepbound both contain tirzepatide made by Eli Lilly. They have separate labels: Mounjaro covers type 2 diabetes, and Zepbound covers weight management and sleep apnea. For weight management, Zepbound is the product labeled for that use.

The Zepbound label states that it contains tirzepatide and that using it with other tirzepatide-containing products is not recommended 2. The two labels share the same starting dose and step-up pattern but list different maintenance doses for each indication 1,2.

Compounded or research-labeled products that say "tirzepatide" are not the same as these approved products; see the legal question below.

What the research shows

What results has tirzepatide shown for body-weight change?

Human studies
In the 72-week SURMOUNT-1 trial, adults with obesity without diabetes had average body-weight changes of -15.0%, -19.5% and -20.9% across three doses. Placebo gave -3.1%. These are group averages from a randomized human trial, not predictions for one person.

SURMOUNT-1 enrolled 2,539 adults 4. Adverse events led 4.3% to 7.1% of tirzepatide participants to stop, versus 2.6% with placebo 4.

Beyond body weight, tirzepatide is labeled for type 2 diabetes and sleep apnea, and Mounjaro for cardiovascular risk reduction in high-risk type 2 diabetes 1,2. Individual photos and stories are not trial evidence.

How long does tirzepatide take to work, and when does it stop working?

Drug label
No individual timeline is established. The Zepbound weight trials lasted 72 weeks, including a dose step-up of up to 20 weeks. They reported results as averages at fixed time points. One-month or three-month results are not label endpoints.

The label describes study lengths of 72 to 88 weeks for weight trials and 52 weeks for sleep-apnea trials 2. Results at a set week are averages; individual responses vary.

The label does not define a point at which tirzepatide "stops working" 2. One Zepbound trial (Study 4) had a 36-week open-label period, then 52 weeks of randomized withdrawal 2. Amiravo's record does not summarise its outcome.

Will tirzepatide lower A1C in type 2 diabetes?

Human studies
Yes, in trials. SURPASS-2 was a 40-week trial in adults with type 2 diabetes. HbA1c fell by 2.01 to 2.30 percentage points across three tirzepatide doses, versus 1.86 with semaglutide. All three doses were superior to semaglutide on this measure.

SURPASS-2 was open-label and included 1,879 participants 5. Serious adverse events were reported in 5% to 7% with tirzepatide versus 3% with semaglutide 5.

The Mounjaro label covers glycemic control in adults and in children 10 years and older with type 2 diabetes 1.

Is tirzepatide approved for sleep apnea?

Drug label
Yes. Zepbound is FDA-approved for moderate to severe obstructive sleep apnea in adults with obesity. In the 52-week SURMOUNT-OSA trials, the apnea-hypopnea index fell by about 25 to 29 events per hour with tirzepatide. It fell by about 5 with placebo.

SURMOUNT-OSA was two randomized, placebo-controlled trials: one in people not using positive airway pressure and one in people using it 6. Mean changes were -25.3 versus -5.3 and -29.3 versus -5.5 events per hour 6.

The label's maintenance dosage for sleep apnea is 10 mg or 15 mg once weekly 2.

Dosage, forms and storage

What is the tirzepatide dosage on the FDA label?

Drug label
Both labels start at 2.5 mg injected under the skin once weekly for 4 weeks. The dose may rise in 2.5 mg steps after at least 4 weeks at each dose. Zepbound maintenance is 5, 10 or 15 mg weekly (10 or 15 mg for sleep apnea); the maximum is 15 mg.
LabelStartStepsMaintenance / maximum
Mounjaro, type 2 diabetes 12.5 mg once weekly+2.5 mg after at least 4 weeksMaximum 15 mg (adults), 10 mg (children 10+)
Zepbound, weight management 22.5 mg once weekly for 4 weeks5 mg, then +2.5 mg after at least 4 weeks5, 10 or 15 mg; maximum 15 mg
Zepbound, sleep apnea 2Same as aboveSame as above10 or 15 mg

The labels state that 2.5 mg is a starting dose, not a maintenance dose 2. Doses are in milligrams, not syringe "units". The label's missed-dose rules and study details are on Tirzepatide dosage in human studies.

Is there an oral tirzepatide tablet?

Drug label
No. Both FDA labels describe tirzepatide only as a solution for injection under the skin, in pens and vials. No oral tirzepatide is approved, and products sold as oral tirzepatide have no label behind them. Any such product is unapproved.

Zepbound comes as single-dose pens, single-dose vials, multi-dose vials and KwikPens, all for subcutaneous use 2. Mounjaro comes in similar presentations 1. For why peptides are rarely taken by mouth, see Why most peptides are injected.

Should tirzepatide be refrigerated, and how long can it stay out of the fridge?

Drug label
Yes. The labels say to refrigerate at 2°C to 8°C (36°F to 46°F) and never freeze it. Single-dose pens and vials may be kept unrefrigerated, up to 30°C (86°F), for a total of 21 days. Multi-dose vials and KwikPens allow 30 days.

Opened multi-dose vials and KwikPens are discarded after 30 days at room temperature or 30 days after first use 2. They are also discarded after 4 weekly doses, whichever comes first 2. Unopened ones kept in the fridge last until the printed expiration date 2.

The labels also say to protect it from heat and light and keep it in the original carton 1,2. These rules apply to the approved products only, not to research-labeled powders.

Side effects and risks

What are the side effects of tirzepatide?

Drug label
The most common side effects are gastrointestinal: nausea, diarrhea, vomiting and constipation, mostly during dose increases. In the Zepbound weight trials, 56% had a gastrointestinal reaction versus 30% with placebo. Injection-site reactions, fatigue and hair loss were also more frequent.
Reaction (Zepbound weight trials)TirzepatidePlacebo
Nausea25% to 29%8%
Diarrhea19% to 23%8%
Vomiting8% to 13%2%
Constipation11% to 17%5%
Injection-site reactions6% to 8%2%
Stopped because of adverse reactions4.8% to 6.7%3.4%

Source: Zepbound label 2. Label warnings include severe gastrointestinal reactions, pancreatitis, gallbladder disease, kidney injury from dehydration and allergic reactions 2. The label advises people on oral hormonal contraceptives to switch to, or add, a non-oral method 2. This applies for 4 weeks after starting and after each dose increase 2.

Does tirzepatide make you tired or cause headaches?

Drug label
Fatigue is a listed side effect: 5% to 7% of people on Zepbound in the weight trials versus 3% with placebo. Headache is not among the label's most common reactions. The label lists headache and dizziness as possible signs of low blood sugar, mainly when combined with insulin or sulfonylureas.

The label's fatigue figure includes reports of weakness, lethargy and malaise 2. Dizziness was reported in 4% to 5% versus 2% with placebo 2.

Low blood sugar is more likely when tirzepatide is used with insulin or an insulin secretagogue 1.

Does tirzepatide cause hair loss?

Drug label
Hair loss is a listed side effect. In the Zepbound weight trials it was reported in 4% to 5% of people on tirzepatide versus 1% with placebo. It was more common in women (7.1%) than in men (0.5%), and the label links it to weight reduction.

These figures come from pooled placebo-controlled trials summarised on the Zepbound label 2. The label does not establish that hair loss is a direct drug effect rather than a result of body-weight change 2.

Can tirzepatide cause depression or anxiety?

Drug label
Depression and anxiety are not listed among tirzepatide's common adverse reactions. The Zepbound label previously carried a warning about suicidal behavior and ideation; FDA removed it in February 2026. Anxiety and mood changes are listed as possible signs of low blood sugar.

The Zepbound label's "recent major changes" section records the removal of the suicidal behavior and ideation warning in 02/2026 2. Anyone with new or worsening mood symptoms on any medicine should talk to their prescriber.

The patient information lists anxiety, irritability or mood changes among low-blood-sugar symptoms 1,2.

Is tirzepatide safe, and can it cause cancer?

Drug label
Tirzepatide is FDA-approved for its labeled uses, and its labels set out the known risks. The boxed warning states that it caused thyroid C-cell tumors in rats; whether it does so in humans is unknown. People with medullary thyroid cancer or MEN 2 history must not use it.

The boxed warning notes that routine calcitonin or thyroid ultrasound monitoring is of uncertain value for early detection 1. Contraindications also include known serious hypersensitivity to tirzepatide 2.

Postmarketing reports include severe pancreatitis, bowel obstruction and kidney failure; the labels state that causality cannot be reliably established from these reports 2. The published trials reviewed lasted up to 72 weeks 4.

How it compares and combines

Tirzepatide vs semaglutide (Ozempic): which is better?

Human studies
In the head-to-head SURMOUNT-5 trial in adults with obesity, average body-weight change at 72 weeks was -20.2% with tirzepatide and -13.7% with semaglutide (Wegovy doses). The trial was open-label. The drugs differ in receptors and labeled uses, so "better" depends on the question.

SURMOUNT-5 enrolled 751 adults without type 2 diabetes 9. In diabetes, SURPASS-2 found larger HbA1c reductions with tirzepatide than with semaglutide 5.

Labeled uses also differ; for example, Zepbound is labeled for sleep apnea 2. The full comparison is on Semaglutide vs tirzepatide.

Tirzepatide vs retatrutide: what is the difference?

Human studies
Tirzepatide is approved and acts on two receptors (GIP and GLP-1). Retatrutide is investigational and adds a third, the glucagon receptor. No trial has compared them directly, so their results come from separate studies. Comparing those separate averages is unreliable because the trial designs differ.

SURMOUNT-1 reported -15.0% to -20.9% at 72 weeks for tirzepatide 4; TRIUMPH-1 reported -17.6% to -25.0% at 80 weeks for retatrutide 11. Differences in design make these numbers hard to compare.

See Tirzepatide vs retatrutide.

Can tirzepatide be stacked with other peptides?

Drug label
The Zepbound label states that using tirzepatide with other tirzepatide products or with any GLP-1 receptor agonist is not recommended. No trial giving tirzepatide together with cagrilintide was found, so any rationale for that pairing is theoretical.

The labeling caution is in the Zepbound prescribing information 2. Cagrilintide is an investigational amylin analogue 13.

What is known about that pairing is on Cagrilintide + tirzepatide. For the wider drug class, see The incretin pipeline in 2026.

Where the evidence stands

What the studies show

  • Tirzepatide is FDA-approved as Mounjaro (type 2 diabetes) and Zepbound (weight management and obstructive sleep apnea) 1,2,3,14.
  • In SURMOUNT-1, average body-weight change at 72 weeks was -15.0% to -20.9% versus -3.1% with placebo 4.
  • In SURMOUNT-5, tirzepatide produced a larger average body-weight change than semaglutide (-20.2% vs -13.7%) 9.
  • Gastrointestinal reactions are the most common side effects, mostly during dose increases 2.
  • Both labels carry a boxed warning for thyroid C-cell tumors seen in rats 1,2.

Still unknown

  • Whether the rat thyroid C-cell tumor finding applies to humans has not been determined.
  • The clinical relevance of tirzepatide's small heart-rate increase is described on the label as uncertain.
  • Outcomes beyond the published trial durations, apart from the cardiovascular outcome trial, are not covered in the sources reviewed.
  • No trial has tested tirzepatide combined with cagrilintide or other peptides; see Amiravo's tirzepatide entry for updates.
  • Compounded and research-labeled tirzepatide products have not been reviewed by FDA for quality or equivalence.
