Retatrutide: Side Effects, Results, Legality and More, Answered

Retatrutide is an investigational once-weekly injectable peptide that activates three hormone receptors: GIP, GLP-1 and glucagon 1,3. Large human trials exist, but it is not FDA-approved, so no label or approved dose exists; see Amiravo's retatrutide entry 3,12.

19 questions answered, reviewed October 24, 2026. Every answer is sourced; the bars show how strong the evidence is.

The basics

What is retatrutide, and what is it used for?

Human studies
Retatrutide (code name LY3437943) is an experimental peptide drug from Eli Lilly that acts on the GIP, GLP-1 and glucagon receptors. It is being tested in humans for obesity, type 2 diabetes, knee osteoarthritis and sleep apnea. It is not approved for any medical use in the US or any other country.

The published trials were funded by Eli Lilly 1,6. The phase 3 programmes are called TRIUMPH (obesity, including people with knee osteoarthritis or sleep apnea) and TRANSCEND (type 2 diabetes) 3,4,5,11.

All of this work is in humans enrolled in controlled trials. Material sold outside those trials is not the trial product. Amiravo's retatrutide entry lists every trial and registry record.

How does retatrutide work, and is it a GLP-1?

Human + preclinical
Retatrutide is a GLP-1 receptor agonist, but not only that. One molecule also activates the GIP and glucagon receptors, so it is called a triple agonist. In obese mice, the glucagon part added extra energy use on top of the reduced food intake driven by the other two receptors.

In cell studies, its activity at the glucagon and GLP-1 receptors was roughly balanced, with relatively more activity at the GIP receptor 6. The energy-expenditure finding comes from mice, not humans 6.

In humans, early studies showed a half-life of about 6 days, which supported once-weekly injections 10. "GLP-3" is not a recognised drug class. An FDA warning letter cites a product marketed as "GLP-3R (retatrutide)" 17.

For the background on single, dual and triple agonists, see GLP-1, GIP and glucagon: from single to dual to triple agonists.

Is retatrutide FDA-approved, and when will it be approved?

No data
No. Retatrutide is not FDA-approved, and no approval date has been set. Phase 3 trials have reported results, the stage before a company can apply for approval. FDA states that retatrutide is not part of any approved drug and cannot be used in compounding.

The 2026 phase 3 publications describe retatrutide as an agent under investigation 3,4,5. The TRIUMPH-1 and TRIUMPH-3 registry records list the trials as completed 8,16.

Any approval would follow an FDA review of a company application. The company's stated filing plans are covered in Amiravo's news report, Retatrutide: Lilly reports TRIUMPH-2 and TRIUMPH-3 phase 3 results.

What the research shows

What results has retatrutide shown for body-weight change?

Human studies
In randomized human trials, average body-weight change ranged from about -9% to -25% depending on dose and population, versus -2% to -5% with placebo. The largest average, -25.0%, came from the 12 mg group of TRIUMPH-1 after 80 weeks in adults with obesity without diabetes.

These are group averages from monitored trials. Individual stories and photos are not trial evidence.

Trial (humans)WhoLengthRetatrutidePlacebo
Phase 2 obesity 1338 adults with obesity or overweight48 weeks-8.7% to -24.2%-2.1%
TRIUMPH-1 32,339 adults with obesity, no diabetes80 weeks-17.6%, -23.7%, -25.0%-3.9%
TRIUMPH-2 41,152 adults with obesity and type 2 diabetes80 weeks-11.9%, -16.8%, -18.8%-5.1%
TRANSCEND-T2D-1 5537 adults with type 2 diabetes40 weeks-11.5% to -15.3%-2.6%

TRIUMPH-1 also reported larger improvements than placebo in knee-pain scores and in the apnea-hypopnea index in its osteoarthritis and sleep-apnea subgroups 3. A 2026 report of sleep-apnea substudies nested within TRIUMPH-1 and TRIUMPH-2 reported the same direction of effect 11.

How long does retatrutide take to work?

Human studies
No timeline has been established for any individual. Trials measured outcomes at fixed points. The phase 2 obesity trial's primary endpoint was at 24 weeks; the phase 3 trials reported at 40 or 80 weeks. One-month results are not a published endpoint.

Trial results describe averages at set time points, not what one person will see 1,3,5. No study has examined why some people report that retatrutide "stopped working" or "is not working".

The longest published treatment period is 80 weeks 3,4. The TRIUMPH-1 registry lists an optional extension to week 104, without published results in the sources reviewed 8.

What does retatrutide do in type 2 diabetes?

Human studies
In people with type 2 diabetes, retatrutide lowered HbA1c (a measure of average blood sugar) more than placebo. In the 40-week TRANSCEND-T2D-1 trial, HbA1c fell by 1.69 to 1.94 percentage points versus 0.81 with placebo. No severe hypoglycemia was reported.

In the phase 2 diabetes trial, HbA1c change at 24 weeks ranged from -0.43 to -2.02 percentage points across doses 2. That compares with -0.01 with placebo and -1.41 with dulaglutide 2. Body weight fell by up to 16.94% at 36 weeks 2.

TRANSCEND-T2D-1 enrolled adults managing diabetes with diet and exercise alone 5. TRIUMPH-2 studied adults with both obesity and type 2 diabetes 4.

Dosage and use

What retatrutide dosage was used in clinical trials?

Human studies
Retatrutide has no approved dosage. In trials it was injected under the skin once weekly. Phase 3 trials tested 4, 9 and 12 mg; phase 2 trials tested 0.5 to 12 mg, some with step-up periods 1,2. These are facts about the studies, not instructions.

In the phase 2 obesity trial, some groups started at 2 mg and stepped up to their target dose 1,7. The phase 3 abstracts do not describe the step-up schedules 3,4,5.

Every trial used fixed doses, not doses by body weight 1,3. Charts "by weight" and dose calculators found online do not come from the trials. The full list of study arms is on Retatrutide dosage in human studies.

Is there an oral retatrutide pill?

Human studies
No oral form appears in the published research. Every phase 1, 2 and 3 study reviewed gave retatrutide as a once-weekly injection under the skin. Products sold as oral retatrutide have no published human data behind them, and no absorption data for an oral form exist in the sources reviewed.

The phase 1b, phase 2 and phase 3 trials all used subcutaneous injection 1,3,10. Peptides are usually broken down in the gut, which is why most are injected; see Why most peptides are injected.

Should retatrutide be taken in the morning or at night, or on an empty stomach?

No data
There is no evidence either way. Trial participants injected once weekly, but the trial abstracts do not mention time of day or meals. Because retatrutide has no FDA label, no official administration instructions exist, and no study has compared timing options.

The trial abstracts describe once-weekly subcutaneous injections and do not mention time of day or food 3,4. Approved drugs in the same family carry these details on their labels; retatrutide does not have one.

Side effects and risks

What are the side effects of retatrutide?

Human studies
Gastrointestinal effects were the most common in human trials: diarrhea, nausea, vomiting and decreased appetite. Retatrutide also raised heart rate in a dose-dependent way in phase 2. In TRIUMPH-2, low blood pressure and altered skin sensation (dysesthesia) were more frequent than with placebo.
Event (TRIUMPH-2, 80 weeks)RetatrutidePlacebo
Diarrhea27% to 34%13%
Nausea14% to 28%8%
Hypotension1% to 6%under 1%
Dysesthesia4% to 7%1%
Stopped because of adverse events or death4% to 12%5%

Source: TRIUMPH-2 4. In the phase 2 obesity trial, nausea affected 45% of the highest-dose group versus 11% with placebo 7. Heart-rate increases peaked at 24 weeks and declined afterwards 1.

A 2026 review notes that skin-sensitivity events were also reported in phase 2, at 7% versus 1% 15. The sources reviewed do not report side effects separately for women and men.

Is retatrutide safe?

Human studies
That cannot be answered with a yes. In monitored trials of up to 80 weeks, the most common adverse events were gastrointestinal, and some participants stopped treatment because of them. Long-term effects are unknown, and products sold outside trials are not the trial material.

Deaths occurred in TRIUMPH-2 and TRANSCEND-T2D-1; investigators judged all of them unrelated to the study drug 4,5. Discontinuation for adverse events was 2% to 5% versus 0% in TRANSCEND-T2D-1 5.

FDA warns that unapproved incretin products labeled "for research purposes" are of unknown quality 12. Trial safety data describe the manufacturer's product given under medical supervision, not research-market vials.

Does retatrutide cause hair loss or make you tired?

No data
Hair loss and tiredness are not among the adverse events reported in the retatrutide trial publications Amiravo reviewed. Full phase 3 adverse-event tables were not available, so this is unresolved. The approved drug tirzepatide lists both hair loss and fatigue on its label.

The TRIUMPH-1 abstract lists gastrointestinal events as the most common 3. Hypotension and dysesthesia were reported in TRIUMPH-2 4. Absence from an abstract does not mean an event never occurred.

For comparison, the Zepbound (tirzepatide) label reports hair loss in 4% to 5% versus 1% with placebo 13. It reports fatigue in 5% to 7% versus 3% 13. Those figures apply to tirzepatide, not retatrutide.

Can retatrutide cause cancer?

No data
The published retatrutide trial reports reviewed do not report on cancer, and the trials lasted at most 80 weeks, too short to settle the question. TRIUMPH-1 excluded people with a personal or family history of medullary thyroid cancer or MEN 2. Approved drugs in this family carry a rodent thyroid-tumor boxed warning.

The exclusion criteria are listed in the TRIUMPH-1 registry record 8. The tirzepatide label's boxed warning describes thyroid C-cell tumors in rats and states that the human relevance is unknown 13.

Retatrutide has no label, so no equivalent warning or long-term safety review exists for it.

Will retatrutide show up on a drug test?

No data
No published data describe retatrutide detection on drug tests, including workplace or clinical panels. In sport, WADA's S0 class prohibits at all times unapproved substances not covered elsewhere on the List. Retatrutide has no approval for human therapeutic use.

S0 covers any pharmacological substance not addressed by any subsequent section of the List and with no current approval for human therapeutic use 9. Retatrutide has no such approval 12. How anti-doping rules treat peptides is covered in Peptides and sport.

How long does retatrutide last in the fridge?

No data
There is no published answer. Retatrutide has no FDA label, so no approved storage temperature or shelf life exists. No published storage data exist for retatrutide outside its trials. General peptide storage principles are in Amiravo's storage guide.

FDA states that retatrutide is not part of any approved drug, so no "how supplied and storage" section exists for it 12. General handling principles are in Storage and handling basics.

How it compares and combines

Retatrutide vs tirzepatide: which is better?

Human studies
No trial has compared them head to head, so neither can be called better. Retatrutide adds glucagon-receptor activity to tirzepatide's GIP and GLP-1 activity. Tirzepatide is FDA-approved; retatrutide is still investigational. Their separate trials used different populations, doses and lengths, so their averages cannot be ranked.

In separate trials, TRIUMPH-1 reported -17.6% to -25.0% at 80 weeks 3 and SURMOUNT-1 reported -15.0% to -20.9% for tirzepatide at 72 weeks 18. Different designs and lengths make direct comparison unreliable.

The full side-by-side is on Tirzepatide vs retatrutide.

Are retatrutide and Ozempic (semaglutide) the same?

Human studies
No. Ozempic contains semaglutide, an FDA-approved drug that activates only the GLP-1 receptor. Retatrutide is a different, investigational molecule that activates GLP-1, GIP and glucagon receptors. No trial has compared them directly, and their separate trials differ in design and length.

The Ozempic label describes semaglutide as a selective GLP-1 receptor agonist 14. Retatrutide's receptor profile is described in its trial reports 3,6.

See Semaglutide vs retatrutide for the full comparison.

Can retatrutide be taken with tirzepatide or stacked with other peptides?

No data
No study has tested retatrutide together with tirzepatide, cagrilintide, MOTS-c or growth-hormone peptides, in humans or animals. Tirzepatide already acts on two of retatrutide's three receptors. No study has tested cycling retatrutide either. Amiravo's combination pages set out the theoretical rationale and open risks for each pairing.

The tirzepatide label states that combining it with any other GLP-1 receptor agonist is not recommended 13. Retatrutide is a GLP-1 receptor agonist 3.

Amiravo's combination pages list what is known and unknown: cagrilintide + retatrutide, retatrutide + MOTS-c and retatrutide + CJC-1295 + ipamorelin. Muscle-preservation claims for these pairings are untested.

Where the evidence stands

What the studies show

  • Retatrutide activates the GIP, GLP-1 and glucagon receptors and is given by weekly injection in trials 1,6.
  • In TRIUMPH-1, average body-weight change at 80 weeks was -17.6% to -25.0% versus -3.9% with placebo in adults with obesity without diabetes 3.
  • In type 2 diabetes, HbA1c fell by 1.69 to 1.94 percentage points versus 0.81 with placebo over 40 weeks 5.
  • Gastrointestinal events were the most common adverse events 1,4. Heart rate rose with dose in phase 2, and dysesthesia and hypotension exceeded placebo in TRIUMPH-2 1,4.
  • Retatrutide is not FDA-approved and cannot be used in compounding, according to FDA 12.

Still unknown

  • Effects and safety beyond 80 weeks of treatment have not been published.
  • Full adverse-event tables for the phase 3 trials, including rates of hair loss and fatigue, were not available in the sources reviewed.
  • The phase 3 abstracts do not describe the step-up schedules; see Amiravo's retatrutide entry for updates.
  • No study has tested retatrutide combined with any other peptide or weight-management drug.
  • Nothing is known about the content or purity of research-market products compared with the trial material.
